We are now offering a Medically Assisted Weight Loss program with specialized diet and activity recommendations and augmented by the use of Semaglutide injections weekly to FINALLY help you reach your goal weight, and quickly. At your initial appointment for a medically assisted weight loss program, several important steps will take place to set you on the right path towards achieving your weight loss goals. Here’s what you can expect:

Assessment and Medical History Review:

You’ll meet with Bridget A Molloy, Board Certified Family Nurse Practitioner, who has worked in Primary care since 2009 and has extensive experiance with the GLP-1 Agonist class (Ozempic, Trulicity, Mounjaro, and Bydureon as well as Wegovy and Saxenda), and will be using Semiglutide (which is a generic Ozempic/Wegovy) in conjunction with lifestyle modifications with diet improvment and increased activity level.

At your Initial visit, I will review your medical history, including any existing health conditions, medications you’re taking, and past weight loss attempts.
I will discuss your weight loss goals and any specific challenges you’ve faced in the past.
Physical Examination:
A physical examination may be conducted to assess your overall health and identify any potential barriers to weight loss.
Vital signs like blood pressure, heart rate, and body measurements will also be taken.
I will also be ordering baseline labs, which you can get through your health insurance through Quest diagnostics or Bioreference Laboratories. If you have to pay out of pocket, the lab work will cost less than $45 total and will be drawn every three months while on this medication.

Personalized Treatment Plan:

Based on your medical history and assessment, I will create a personalized treatment plan tailored to your specific needs and barriers to make it so you will be as successful as possible with this plan.
The plan includes dietary recommendations, exercise guidelines, and other lifestyle modifications.

Discussion of Options:

You’ll learn about different medically supervised weight loss options available to you.
These options may include:

Calorie-Controlled Plan: Involves healthy food choices based on your preferences.

Food and Liquid Meal Replacement Plan: Incorporates liquid meal replacement shakes alongside regular meals.

Intermittent Fasting

Prescription Medications for Weight Loss: If appropriate, I will recommend, in conjunction with lifestyle modifications (diet improvement/changes and increased activity level as tolerated), the use of a once weekly GLP-1 Agonist injection, Semaglutide (if clinically appropriate). You can come in weekly for a weight, and an injection by Bridget, or you can take all four doses/syringes home with you and administer yourself or have a family member administer. (* you will be taught how to appropriately administer this medication. I buy the most expensive, and smallest gauge Insulin syringes available to reduce discomfort with the injection, and bruising (which is rare and minimal if occurs.

First month dosing
You will start on Semiglutide 0.25mg, injected once weekly and will be given your first injection in office. You will stay on this dosage for 1 mo. First month fee is: $400, due at your first visit. I accept Cash, and can easily process any credit cards.

Month 2- we increase the dosage of the medication, Semiglutide to 0.50mg to be injected subcutaneously once a week for 1 month- Price will vary depending on dosage.
Dosages of 0.50 and 1mg injected once weekly in months 2&3 will increase to $450/mo due to price of the medication. (higher dosage=more expensive)
We will increase dosage every month for 5 months and the highest you will pay a month is $550/mo for doses 1.75mg and above (1.75mg & 2.5mg)

Follow-Up Appointments:

You’ll schedule follow-up appointments with Bridget molloy, to monitor your progress once monthly or weekly if you choose not to inject yourself- I am happy to do this and there is no extra fee for me to inject weekly. If you choose to come in once a week we will also check your BP, HR and weight at that visit, which will take less than 10 minutes once a week and my office is located at Polished from head to toe in Peru, NY.

**IMPORTANT** THE MONTHLY INCREASE IN DOSAGE IS ONLY A TREATMENT RECOMMENDATION. MANY CLIENTS DO NOT NEED A DOSAGE HIGHER THAN 0.25-0.50MG TO REACH THEIR GOAL WEIGHT, MAINTAIN AND EVENTUALLY GET OFF THIS MEDICATION ALLTOGETHER. YOU WILL NOT BE ON THIS MEDICATION FOR A LIFETIME. I FEEL THAT USING LOWEST DOSE OF ANY MEDICATION, THAT CAN GIVE US OUR DESIRED RESULTS/OUTCOMES IS OF UTMOST IMPORTANCE AND MY CLIENTS ARE NOT REQUIRED TO DO THE MONTHLY DOSAGE INCREASE AND WILL ASSESS YOUR PROGRESS AT EVERY VISIT.

These appointments allow adjustments to be made to your treatment plan as needed.

I am very excited to now offer Tirzepatide injections and am able to treat clients with Tirzepatide OR Semaglutide (AKA Mounjaro- Tirzepatide and Wegovy- Semaglutide) both GLP-1 Agonists and work on the gut, by slowing gastric emptying, therefore, food and liquid stay in your stomach for longer periods of time, which then triggers your brain and tells you that you are full/ satiated. These medications assist one in reducing their hunger drive, and clients end of eating a quarter of what they used to eat. Tirzepatide (AKA Mounjaro) not only Activates the GLP-1 and GIP receptors in the stomach and with the metabolizing of glucose. In my experience, those who experience nausea/vomiting easily with medications, Tirzepatide is a much better option for you given that the SE profile for gastrointestinal upset is much less than Semaglutide and I do think it is due to Tirzepatide also hitting the GIP receptor in the gut.

Taking either Medication as prescribed will not cause hypoglycemia. However, if you do not eat a small breakfast, lunch and dinner with small snacks in between,, you are at risk for your blood sugar to drop, and this happens to all of us if we do not eat for long periods of time. GLP-1 Agonists have been on the Market since 2004, they are well studied and have incredible health benefits - PREVENTING disease and helping to Protect the heart and decrease risk for cardiovascular events. They also aid one in losing weight which can thereby reduce any joint pain that you may be having.

This is a medically monitored weight loss program, you will be meeting with me regularly, Bridget Molloy, FNP-BC and am a Board Certified Family Nurse Practitioner. Weight loss with Tirzepatide as an adjunct to lifestyle modifications with diet improvement and increased activity level. Tirzepatide dosing- 2.5mg (month 1), 5mg (month 2), 7.5mg (month 3), 10mg (month 4) and 12.5mg (month 5 and onward)- Starting dose of 2.5 $400/month and will increase $50 with each dosage increase. I do have promotions often and will not always be $400 + a month and price is subject to change (decrease or increase) based on how much I can buy the product for. Tirzepatide is a GLP-1 Agonist, which also hits the GIP receptor which Semaglutide does not which helps to further improve satiety. Overall, most seem to have less side effects on Tirzepatide (AKA Mounjaro), therefore if you have taken Semaglutide (AKA Wegovy / Ozempic) in the past, and had significant Side Effects, then Mounjaro may be a better option.
